Forms Generator Tool (FGT) provides the ability to instantly generate formatted electronic reports based on Staged Electronic Data Deliverable (SEDD) files. FGT allows you to generate Contract Laboratory Program (CLP) forms as well as a generic form that can be used across programs.
To begin, click Browse in Step 1 and locate your SEDD XML file or Zip file. Select the type of forms you would like to generate in Step 2 and click Populate Forms. A list of forms applicable to the file you uploaded will be provided, click the Preview/Print icon to view, save, or print the form. Please note FGT is currently supporting SEDD Specifcations 5.0 and 5.1 Stage 2a and Stage 3 as well as Specification 5.2 Stage 2b. FGT also provides a Generic Form 1 for use with any SEDD based file and CLP QC Summary reports. The following is a summary of supported forms for each SEDD Stage:
